Head to Toe is a family-focused program which provides children and their families with education and instruction about important lifestyle habits and weight management. Therefore, program facilitators need to collect information and consent forms for each participant (including adult participants). It is important for us to have certain background and medical information concerning all participants (parents and children) so that you can fully participate in our program. All forms, except for the Kids Assessment and the Physician Release, should be filled out by a parent.
