Volunteer Application

Thank you for attending a Volunteer Information Session.  You have now completed 1 of 4 steps of the application process. To apply for a St. Louis Children's Hospital volunteer position, please complete ALL three steps below:

Step 2: Click here to complete the online volunteer application.

Steps 3 and 4: Print two copies of the reference form, which is available here, and the Criminal History Background Check release form, which is available here. The reference forms must be completed by non-family members (i.e. former/current employer, teacher/professor, school counselor, coworkers, etc.). Please submit all forms via fax or mail them in one envelope to the Volunteer Services Office:

Once we have received and reviewed both the online application and Reference and Criminal History forms, you will be contacted with information regarding the next step*. If your schedule availability and volunteer interests match our current needs, you will receive information via e-mail or phone about upcoming New Volunteer Orientations. If we do not have a need for additional volunteers during your specified time of availability, you will receive a post-card in the mail noting your options.

*Note: You will not be contacted by Volunteer Services until your volunteer application has been reviewed. Your volunteer application will not be reviewed until you have attended a mandatory informational session and have submitted your online application, reference forms and criminal history form.

    Information regarding Volunteer Health Requirements
    If you are accepted as a volunteer candidate, you are required to supply the following documentation when you attend the New Volunteer Orientation.

    • Current Immunization Record from physician. New volunteers are also required to provide documentation of TB test results from the current year of submitting an application as well as a seasonal flu shot during Flu Season.
