Before your visit

• We will send you a visit package including maps, a schedule, a developmental questionnaire and registration information.
• We will also contact you to perform a brief interview to answer questions and identify areas of concern.

On the day of your visit
Please bring the following:

• Your visit package with completed developmental questionnaire.
• Copies of any new medical informationList of any questions

After your visit
After your visit you will receive a comprehensive written review of the visit.  A summary letter will also be sent to your pediatric primary care provider.
