Supporting children with congenital heart disease (CHD) by teaching them about their hearts as they grow

The Transition Education for Adolescents with Congenital Heart (TEACH) Clinic ensures patients remain in cardiology care and become knowledgeable self-advocates who actively participate in their own heart care. We also guide and empower caregivers to continue transition education at home.

Who should participate?

  • Children and adolescents with congenital heart disease, ages 12–22
  • Children and adolescents with an intellectual disability who are not expected to achieve independent self-care in adulthood are also eligible to participate. The focus is on helping caregivers navigate the evolving medical and legal landscape as patients age into adulthood.

Services offered

We recommend three TEACH visits throughout adolescence, beginning at age 12. We offer in-person and telemedicine visits in Missouri and Illinois. During these visits, we:

  • Educate kids about their heart and surgeries, using personalized heart diagrams
  • Teach kids how heart issues connect with other health issues
  • Help kids understand and navigate the complex health care system
  • Guide parents and caregivers on how to help their kids become more independent

Transitioning from pediatric to adult care

The final TEACH Clinic introduces patients to team members at the Center for Adults with Congenital Heart Disease (ACHD) at the WashU Medicine and Barnes-Jewish Heart and Vascular Center to ensure a seamless transfer of care.

Frequently asked questions

Will my child still see their normal cardiologist?

Yes. TEACH visits are extra support and will not change the patient’s care plan or doctor. Patients, TEACH staff, and the primary cardiologist will decide the right time to move to adult heart care, usually between the ages of 18–22.

How can I support my child in taking more ownership of their heart health?

  • Encourage your teen to do most of the talking during doctor visits and complete their own forms
  • Have your teen track their doctor appointments on their phone or calendar
  • Bring your teen along for medication refills
  • Access more educational resources at the Adult Congenital Heart Association (ACHD) website
  • Visit the Prepare™ website to learn about appropriate medical directives
