The Healthy Kids Express™ - Asthma Program is made up of a team of trained professionals to help each child receive the best asthma care possible.
Nurse practitioners: Provide asthma exams, that include a special test to measure lung function, create an Asthma Action Plan for the child and partner with the child’s primary care provider for continuity of care.
Nurses: Work one-on-one with children teaching them how to take their asthma medicines and recognize and manage their symptoms.
Asthma Coaches: Give year-long support to caregivers and their children with asthma.
Asthma Health Educators: Provide education and asthma awareness at community events.
Social Worker: Links families in need of support to helpful community resources.
