2019 Summer Camp Independence
DATES: June 10-14, June 17-21, June 24-28
July 8-12, July 15-19, July 22-26
TIME: 9:00 am to 4:00 pm
LOCATION: Webster Groves Recreation Complex, 33 E. Glendale, Webster, MO
AGES: Between the ages of 7-18 years old, functioning at a first-grade level or higher and able to follow directions. Campers must also be able to sit independently or with minimal support and have reasonable use of at least one arm.
ACTIVITIES: Camp activities will include adaptive basketball, dance, soccer, martial arts, baseball and ice skating. Participants will receive specialized attention from pediatric therapists in adaptive sports programs designed to challenge campers to increase their activity level, aerobic fitness, strength and balance in a safe, fun and rewarding atmosphere. Our program will provide campers the opportunity to experience sports in a whole new dimension. Our goals are to offer a wide variety of adaptive sports options to keep individuals with CP active for life and to help them embrace the concept that health and physical fitness are possible and necessary in order to maximize INDEPENDENCE.

The following forms are required to attend 2019 Summer Camp Independence:
- Camper Application and Information
- Seizure Action Plan, Food Allergy and Anaphylaxis Emergency Care Plan, Medication Administration Form
- Media Release Form
Volunteer at Summer Camp Independence
If you would like to volunteer at 2019 Summer Camp Independence, please complete the following steps outlined below.
- Two Reference Forms
- Criminal History Background Check Release Form
- Background Check Authorization Form (complete top half of the Authorization Form only)
- Parental Consent Form (only to be completed for volunteer candidates younger than 18 years old)
Step Three: Submit documentation of the following required immunizations.
- 2 MMR (Measles, Mumps and Rubella) vaccines
- 2 Varicella (Chicken Pox) vaccines or proof of having Chicken Pox from a physician
- Tdap (Tetanus, Diphtheria, Pertussis)
- Current TB test (placed and read within the last year to date)
** Following submission and review of your immunizations records, a TB test may be completed at St. Louis Children's Hospital Occupational Health free of charge. All other immunizations may be completed at your primary physician’s office. If needed, instructions to do so will be provided by Camp Coordinator, Deb Quarells.
Step Four: Return all of completed required forms and immunizations records at one time to St. Louis Children’s Hospital, Volunteer Services, via fax, e-mail (as a PDF) or mail.
*All required forms must be sent by the applicant at one time. We will not accept incomplete forms or forms sent separately. If the required forms are not sent at one time, the individual’s application packet will be considered incomplete and will not proceed through the review process.

Step Five: Attend New Volunteer Orientation
After review of your Camp Independence volunteer application, if you are selected as a volunteer you will receive an email invitation to attend Camp Independence Volunteer Orientation on Wednesday, June 5, 2019, 6:30 – 7:30pm. This email will include a link to RSVP. You must RSVP to confirm your attendance. If you are unable to attend this date, you may receive an orientation on the first day you volunteer.
Step Six: Camp Independence Volunteer Placement
Following confirmation of a clear background check and verification of the required immunizations, you will receive an email confirming your volunteer dates at Camp Independence including instructions for your very first day.
