For your child’s first visit to the Spina Bifida Clinic, you should bring:

  • Completed health history and medical information
  • Medical records, X-rays and radiologic studies
  • Current insurance card and photo identification
  • Visit referral, if required by your insurance
  • Social Security number of patient and parent
  • A list of spina bifida questions you or your child may have
  • Any equipment your child uses (e.g., braces, wheelchairs, walkers)

Once your child has met with the clinic’s doctors, therapists and nurse, they will provide you with a treatment plan overview. They also will review the timeline for continuing care of your child.
